Important Notice: Clinic Closure (December 2024)

Dear Dakota Child and Family Clinic Patient,  

We are writing to inform you that the Open Door Health Center Dakota Child and Family Clinic (DCFC) will be closing its doors permanently on December 30, 2024. You will need to establish a new health care home or transfer your care to one of our Mankato-based providers to continue receiving care. We recommend you contact your health insurance company for a list of covered providers. We also have a list of additional resources located at the bottom of this page.

This decision was made by the Open Door Health Center Board of Directors and was not made lightly.  The staff and board of ODHC deeply appreciates the trust you placed in DCFC staff to provide you and your family with quality healthcare over the years. We know that this will cause anxiety, concern, and sadness for many of you, and we commit to providing you with transparency to the extent we are able to do so.  

In August 2023, DCFC, faced an uncertain future with significant financial challenges. Thanks to an infusion of cash from generous individual donors and a reduction in hours of operation, the clinic was able to keep its doors open while the Board of Directors and leadership pursued a merger opportunity to avoid closing the clinic doors permanently. In October 2024, the merger with Open Door Health Center (ODHC) was completed.   

Mergers are complex and often require strategic compromise. As stewards of our organization, we must ensure that we have all the necessary legal, ethical, financial, and governance policies and procedures in place to ensure the long-term success of the organization so that we can continue to serve our patients across the region. Due to workforce challenges, inflation, and healthcare market economics, we are unable to keep the clinic location open. We know this news is disappointing. We at Open Door Health Center are here to assist you with next steps and ensure continuity of care. 

To transfer your records to a new provider or obtain a personal copy, please complete a Release of Information (ROI) and return it to our office at 2530 Horizon Drive, Burnsville, MN 55337. You must complete a separate ROI for each individual.  After January 31, 2025, your health records will be available at Open Door Health Center and can be requested at any time by calling 507-388-2120. Requests will be filled as per time limitations dictated by state law. 

If you have any questions or concerns, please reach out to the DCFC clinic location by calling 651-209-8640 through January 31, 2025.  After January 31, 2025, please call the ODHC main number at 507-388-2120.

  1. Decide where you would like to transfer care. A list of clinics can be found at the bottom of this page.
  2. Complete a Release of Information (ROI) and submit it to Dakota Child and Family Clinic to transfer your medical records. If you are submitting a physical copy, please fax it to 651-209-8690 or drop it off at DCFC. If you would like to complete an ROI online, please select from these links:

  3. You can access your patient portal in Athena HERE to submit medication refill requests until the end of the year, or call the prescription line.

*Your medical records will continue to be available after January 31, 2025. Requests will be fulfilled with Open Door Health Center beginning on February 1, 2025.
